Albright LIFE

Albright LIFE is a Medicaid and Medicare authorized provider of the Program of All-Inclusive Care for the Elderly (PACE), known in Pennsylvania as LIFE. AlbrightLIFE provides comprehensive medical care and supportive services for adults age55 and older, helping them remain safely living at home for as long as possible. The Albright LIFE Program serves as an alternative to nursing home care by coordinating all aspects of a participant’s care.

For individuals who qualify for Medicaid and live in the community, there are no out-of-pocket costs for services. Those who do not qualify for Medicaid may choose to privately pay.

*Each participant receives a personalized care plan tailored to their unique needs. Services may include primary and specialty medical care, nursing, therapies, home care, medications, nutritional counseling, durable medical equipment, and other medically necessary supports.

*LIFE participants may be responsible for the cost of services that are not approved by the care team. Emergency services are covered whether received from in-network or out-of-network providers. Participation in LIFE is voluntary. Participants may choose to disenroll at any time.

Covenant Home Care

“Covenant Home Care” is a 1 to 1 geriatric home care agency with unique opportunities around the Lancaster and Lebanon areas. In addition to helping seniors in their private homes, Covenant operates within senior living communities, providing assistance to their residents to help them Age in Place!

When to refer: Home care should be considered when there are signs of declining physical or cognitive health, frequent falls, difficulties with daily living activities (bathing, eating), or if the primary caregiver is experiencing burnout. It is also recommended after hospitalization or to manage chronic conditions at home.

Wellness Merge Physical Therapy

Wellness Merge Physical Therapy provides personalized in-home outpatient physical therapy for seniors, as well as in-clinic care for all age groups. Our one-on-one approach focuses on improving strength, balance, mobility, and overall function to help patients stay independent and active.

We feature the Merge Mobility Track, an advanced overhead harness system that allows for safe and effective balance and gait training while reducing fall risk and building confidence.

When to Refer:

  • Fall risk or balance deficits
  • Post-surgery, injury, or hospitalization
  • Neurological conditions (e.g., Parkinson’s, stroke)
  • Need for in-home therapy
  • Decline in mobility, strength, or walking confidence
